- Employment history for both parents for the last year, if known
- Visitation history of the noncustodial parent for the last year

- List of all debts that are community property (mortgage, car loans, credit cards, student loans, etc.)
- List of all insurance and payments on the policies (medical, dental, eye, life, home, car, etc.)
- List of all sole and separate property (gifts, inheritances, etc.)
- Employment history for both parents for the last three years, if known
Estate Planning
- List of all assets that are community property
- List of debts that are community property
- List of all assets that are sole and separate property
- List of all debts that are sole and separate property
- Child's name, date of birth, Social Security number
- An idea of how you want your property distributed upon your death
- An idea of how you wish to handle end of life situations such as irreversible coma, life threatening illnesses, etc.
- Whether or not you wish to be an organ donor
- The name of the person(s) you want to handle your financial and health care decisions when you are incapacitated
